We work with Landlords and Housing Providers to provide housing-related and life skills support to young adults with the aim of developing essential living skills, resilience and increasing independence, to maintain their tenancy themselves.

The Young People placed with our service have the opportunity and participation in getting their voices heard in the planning, implementation, delivery and review of services. The purpose of our service is to enable vulnerable adults to develop existing skills or acquire new skills to increase their independence in daily living through appropriate risk taking, identify and enable opportunities for education and employment, and ultimately meet their desired outcomes as detailed within their Personal Plan. In addition services focus on keeping individuals healthy and safe.

Respite care means taking a break from caring, while the person you care for is looked after by someone else

We at Optimum Healthcare provide short-term respite accommodation as well as care, support and stimulation to people who are usually supported by family carers at home. This support can be provided in their own home or in specialised emergency accommodation.

We have a range of accommodation options available, from shared homes to private rooms, all of which are well maintained and furnished to a high standard. Our on-call team is available 24/7 to assist with urgent referrals.

These services will deliver outcome-based support to individuals who have a range of disabilities and health conditions. This may include:

  • A Learning Disability
  • A Physical Disability or condition
  • Autism
  • A Sensory Impairment, including those with dual sensory impairment

We supply fully trained and checked healthcare workers temporarily to hospitals, nursing and residential homes . Our nurses and healthcare professionals can be booked on an ad-hoc or ongoing basis, 24 hours a day, 7 days a week.
